Forms

For Relocation, you will need the following forms.

Homeowners can apply for funds to either relocate their mobile home or to abandon their mobile home. Please visit the Eligibility Page to determine if you are eligible to apply before beginning the application process.

When you are ready to apply, you can either apply online by clicking on the Application Form for either Relocation or Abandonment and follow the instructions. Alternatively, you can download the forms, complete them, and mail or email them along with the required documentation to FMHRC.

Florida Abandonment Forms

For Abandonment, you will need the following forms:

Acknowledgment by Park Owner When a Home is Abandoned
